    I hereby authorize Reach Fiber Network and its agents to transfer service from my current provider to Skyetel and to transfer my current telephone number(s) used to provide service so that Skyetel may provide its network service to me. I also authorize Skyetel to obtain billing information, customer service records, and other information required to provide me with service on the Skyetel network.

    In addition to above, I agree to the following porting terms:

    1. There may be a fee associated with my port in request.
    2. Port in fees are not refundable.
    3. The information provided above is accurate to the best of my knowledge.
    4. FOC Date changes may only be done once and must be requested two or more business days prior to the FOC date.
    5. Port requests may not be cancelled on their FOC date.
    6. Any ports that are cancelled within two business days of their FOC date incur a $10/order + $2/TN fee.
    7. Any phone numbers that are snapped back or slammed will incur a $300 fee per number.
    8. I want a Port-Out protection PIN added to my numbers (where possible) to protect them from unauthorized Port-Out requests.
    9. Once your number finishes porting, usage and feature costs will begin billing immediately.
